Eyelid hygiene is not one routine. What a mildly gritty eye needs at the end of a screen day and what a crusted, inflamed lid margin needs are different in duration and in order of operations, and applying the wrong one is why people conclude it does nothing.
Mild: occasional grittiness, no visible lid change
Symptoms that come and go, worse after screens or air conditioning, with lids that look normal in the mirror.
Once daily, at night. Warm a clean cloth and hold it against closed lids for a minute or two. Then, with a clean fingertip, wipe gently along the lash line — along it, not across it, and along the margin rather than the eyeball. That is the whole routine. Two minutes.
The temptation is to skip it on days when the eyes feel fine. Those are the days it is doing the most.
Moderate: daily symptoms, visible margin changes
Redness along the lid margin, crusting on waking, a persistent gritty or burning feeling most days, or recurrent styes.
This is where order matters. Heat first — sustained warmth long enough to soften the oil in the glands, which is around ten minutes at a stable temperature rather than ninety seconds under a tap. Then clean, while everything is still warm and mobile. Then, at night, a lid margin oil to work on the mite and bacterial load overnight.

Stubborn: months of symptoms, no real change
You have been doing the moderate routine consistently for eight weeks and the margin still looks and feels the same.
Two things are worth checking before escalating the routine further. First, is the heat actually reaching temperature? Second, is there a Demodex component that needs a longer course than you have given it?
Beyond that, this is the point at which an examination earns its cost. An optometrist can evert the lid and look at the gland openings directly, which tells you in seconds what months of guessing will not.
Why consistency beats intensity
The lid margin does not respond to effort in the way a muscle does. It responds to the accumulated effect of repeatedly clearing debris and keeping openings patent, which means the routine that survives is worth more than the routine that is thorough.
Two minutes every night for six weeks reliably outperforms ten minutes nightly for a fortnight followed by nothing. If you are choosing between doing it properly and doing it at all, do it at all.
