The most common way a successful MGD routine fails is that it works. Symptoms settle, the daily ten minutes starts to feel unnecessary, it slips to occasional, and some weeks later the eyes are gritty again. This is predictable, and it is avoidable with a maintenance phase that is planned rather than drifted into.

Why the glands do not stay fixed

Clearing an obstructed gland removes the blockage. It does not change the thing that caused the blockage — meibum that is more viscous than it should be, openings that keratinise over, a lid margin that inflames easily, or an environment that keeps evaporating your tear film. Those persist. Regular heat and expression keep the flow going in spite of them, and when the input stops the drift back begins.

There is also a floor to what maintenance can achieve. Glands that have atrophied do not regenerate, as whether meibomian glands grow back explains. The aim of maintenance is to keep the glands you still have patent and productive, which is both a modest goal and the one that matters.

When to move from treatment to maintenance

Not by the calendar. Move when symptoms have been stable and acceptable for about four consecutive weeks — not improving, stable — and when expression still yields reasonably freely. If you are still having bad weeks, you are still in the treatment phase, and reducing now simply buys a relapse.

How to taper

Reduce the frequency of heat while keeping each session full length. A session that is cut to four minutes is not a smaller dose; it is largely a wasted one, because the first few minutes are spent getting the tissue to temperature.

  • Step one: drop from twice daily to once daily for a fortnight. Keep lid hygiene daily.
  • Step two: if symptoms hold, drop to four or five sessions a week for a fortnight.
  • Step three: settle at whatever the lowest frequency is that keeps you comfortable. For many people this is two to three sessions a week; for more advanced MGD it stays daily, and that is a legitimate endpoint rather than a failure.

Reading a relapse

Symptoms returning is information about timing, not a verdict on the routine.

  • Within a week of reducing: you were not yet stable. Return to the previous frequency for another month before trying again.
  • After several weeks: the taper went one step too far. Go back one step and stay there.
  • Despite unchanged maintenance: something else has changed — a new medication, a dry winter indoors with heating, a period of heavy screen work, a seasonal rosacea flare, or a mite population that has built back up. Treat the trigger rather than escalating the mask.
  • Progressively worse over months despite consistent maintenance: have the glands reassessed. Structural loss changes what maintenance has to look like.

The parts that work quietly

Maintenance is also where the slow modifiers earn their place, because they act on the composition of meibum rather than on a single blockage. Omega-3 is the main one, with an honest timeline of about three months before any effect and a mixed evidence base that omega-3 for MGD sets out. Keeping humidity reasonable, taking screen breaks and managing rosacea if you have it do more over a year than any intensification of the mask.
